Surveys may contain sub-questions which respondents will only see if they select the answer to which the sub-question is attached. To add a sub-question to a question answer:
- Go to the “Define surveys” section:
- Click the “EDIT SURVEY” button:
- Click the sub-question button next to the answer for which you would like to add a sub-question:
- Drag one of the question types into the box (in this case a YES/NO question has been selected):
- Enter a question in the allotted field and use the “ASSOCIATED RISKS” dropdown menu to relate the question to a risk then select the “Answers” tab:
- Choose a risk score from the drop-down menu and click the document button next to answers for which you would like the respondents to upload a document. Finally, select the “Recommendations” tab:
- Add risk recommendations for each question if desired:
- Click the “SUBMIT” button:
If a respondent selects the “YES” answer for the survey’s first question, they will then be asked to answer the sub-question created here.
