To complete a Survey offline, you will need to contact the Administrator who is managing the program and request the Import Template. Follow these steps to identify the name of the Import Template you need, how to fill in the template, upload and submit it against one or more Surveys at a time.
Step 1. Access My Surveys
1.1 Log in.
1.2 Click MY SURVEYS.
1.2 Click MY SURVEYS.
Step 2. Locate Survey
2.1 Click OPEN FILTER.2.2 Click on the Entity Search FIELD.
2.3 Begin typing the name of the Entity to narrow the options in the drop-down menu or simply click on the NAME of the Entity you wish to filter Survey results for.
2.4 You can filter by Schedule Status by clicking on the Status FIELD and selecting one of these options by clicking on the STATUS NAME.
2.6 When all filters are removed, user can scroll through the complete list of Surveys assigned to you and locate the Schedule name manually.
2.7 Contact your Program Administrator to request the "Import Template" and specify which Schedule.
Step 3. Complete Import Template
3.1 Once you receive the offline template, open it in Excel.
3.2 Template tab houses columns for the Survey name, Survey question, as well as the fields to paste your answer, and any applicable notes.
TIP: ensure that these column headers remain in lowercase for a successful import
3.3 Possible Answers tab houses columns for Survey name, Survey question, question format, possible answers and answers that require supporting information.
3.4 Identify the question you wish to answer ("Template" tab → Column B "Question").
3.5 Click POSSIBLE ANSWERS tab.
3.6 Identify the same question you wish to answer ("Possible Answers" tab → Column B "Question").
3.7 Acknowledge the question type by reviewing Column C "How many can I choose?".
3.8 Review the "Possible Answers" available to respond to your question with ("Possible Answers" tab → Column D "Possible Answers").
3.9 The response options available are separated by a pipe "|", copy the applicable answer(s) but leave out the "|" unless your response requires and permits more than one response to the question.
3.10 Copy the applicable answer from the "Possible Answers" column.
3.11 Click the TEMPLATE tab.
3.12 Confirm that you have identified the corresponding question in Column B "Question".
3.13 Paste the response in Column C "Answer".

3.11 Click the TEMPLATE tab.
3.12 Confirm that you have identified the corresponding question in Column B "Question".
3.13 Paste the response in Column C "Answer".

3.14 Address all Survey responses that require notes by entering clarifying details in the "notes" column (Template → Column D "Notes").
3.15 Save the offline template onto your system in CSV or Excel format only.

Step 4. Bulk Import
4.1 Locate the Survey again in the Prevalent tool.
4.2 Click the TOP CHECKBOX to "Select All for Bulk Upload".
4.3 Click UPLOAD
4.4 Default setting is to "Fill Unanswered Questions Only".
4.5 Click the TOP RADIO BUTTON to select "Overwrite All Current Answers", which will remove any content already submitted when you confirm upload in Step 4.7.

4.6 Drag and drop the template or click BROWSE FILES to select file manually.
4.7 Click CONFIRM to upload or CANCEL to close.
Step 5. Complete Survey
5.1 Review the upload by clicking RESUME on each of the Surveys you performed the bulk upload on.
5.2 Responses requiring your attention have red lines such as Documents Required or Notes Required in the "Answer" column.
5.3 Click EDIT to access the question and provide the missing information.
5.4 Click SAVE & NEXT.
5.5 Questions requiring responses are in red text, click EDIT.
5.6 Respond to the question, click SAVE & NEXT.
5.7 Click BACK TO SUMMARY.
5.8 Once all responses have been completed, the "Submit Survey" button will have a green outline, click SUBMIT SURVEY to submit.
